About Levothyroxine Sodium: Levothyroxine is used to treat an underactive thyroid (hypothyroidism). Show more ▾

It replaces or provides more thyroid hormone, which is normally produced by the thyroid gland. Low thyroid hormone levels can occur naturally or when the thyroid gland is injured by radiation/medications or removed by surgery. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke. Insulin glargine is a man-made product that is similar to human insulin. It may also be used alone or with other diabetes drugs. This monograph is about the following insulin glargine products: insulin glargine, insulin glargine-yfgn. About Symbicort: This product is used to control and prevent symptoms (wheezing and shortness of breath) caused by asthma or ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

It contains 2 medications: budesonide and formoterol. Budesonide belongs to a class of drugs known as corticosteroids. It works by reducing the irritation and swelling of the airways. Formoterol belongs to the class of drugs known as long-acting beta agonists. It works by relaxing the muscles around the airways so that they open up and you can breathe more easily. Controlling symptoms of breathing problems can decrease time lost from work or school. When used alone, long-acting beta agonists (such as formoterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination inhaled corticosteroid and long-acting beta agonists, such as this product, do not increase the risk of serious asthma-related breathing problems. Before using this medication, it is important to learn how to use it properly. If an asthma attack occurs, use your quick-relief inhaler (such as albuterol, also called salbutamol in some countries) as prescribed. See also How to Use section. A pharmaceutical preparation of budesonide and formoterol fumarate that is used as an ANTI-ASTHMATIC AGENT and for the treatment of CHRONIC OBSTRUCTIVE PULMONARY DISEASE.

About Insulin Lispro Protamine Mix: Insulin lispro protamine/insulin lispro is used along with a proper diet and exercise program to control high blood sugar in people with diabetes. Show more ▾

This product is a combination of two man-made insulins: intermediate-acting insulin lispro protamine and rapid-acting insulin lispro. This combination starts working faster and lasts for a longer time than regular insulin. Insulin is a natural substance that allows the body to properly use sugar from the diet. It replaces the insulin that your body no longer produces, thereby lowering your blood sugar. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke.

In 2023, this provider wrote 13 Medicare prescriptions for Insulin Lispro Protamine Mix, about 87% of the volume of the typical South Carolina prescriber of this medication (15). Medicare paid an average of $170.94 per fill, versus $317.67 statewide.

About Acetazolamide: Acetazolamide is used to prevent and reduce the symptoms of altitude sickness. Show more ▾

This medication can decrease headache, tiredness, nausea, dizziness, and shortness of breath that can occur when you climb quickly to high altitudes (generally above 10,000 feet/3,048 meters). It is particularly useful in situations when you cannot make a slow ascent. The best ways to prevent altitude sickness are climbing slowly, stopping for 24 hours during the climb to allow the body to adjust to the new height, and taking it easy the first 1 to 2 days. This medication is also used with other medications to treat high pressure inside the eye due to certain types of glaucoma. Acetazolamide belongs to a class of drugs known as carbonic anhydrase inhibitors. It works by decreasing the production of fluid inside the eye. It is also used to decrease a buildup of body fluids (edema) caused by heart failure or certain medications. Acetazolamide can work less well over time, so it is usually used only for a short period. It has also been used with other medications to treat certain types of seizures (petit mal and unlocalized seizures). One of the CARBONIC ANHYDRASE INHIBITORS that is sometimes effective against absence seizures. It is sometimes useful also as an adjunct in the treatment of tonic-clonic, myoclonic, and atonic seizures, particularly in women whose seizures occur or are exacerbated at specific times in the menstrual cycle. However, its usefulness is transient often because of rapid development of tolerance. Its antiepileptic effect may be due to its inhibitory effect on brain carbonic anhydrase, which leads to an increased transneuronal chloride gradient, increased chloride current, and increased inhibition. (From Smith and Reynard, Textbook of Pharmacology, 1991, p337)

In 2023, this provider wrote 12 Medicare prescriptions for Acetazolamide, about a quarter of the volume of the typical South Carolina prescriber of this medication (43). Medicare paid an average of $25.07 per fill, versus $90.63 statewide.

About Hydrochlorothiazide: This medication is used to treat high blood pressure. Show more ▾

Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. Hydrochlorothiazide belongs to a class of drugs known as diuretics/water pills. It works by causing you to make more urine. This helps your body get rid of extra salt and water. This medication also reduces extra fluid in the body (edema) caused by conditions such as heart failure, liver disease, or kidney disease. This can lessen symptoms such as shortness of breath or swelling in your ankles or feet. A thiazide diuretic often considered the prototypical member of this class. It reduces the reabsorption of electrolytes from the renal tubules. This results in increased excretion of water and electrolytes, including sodium, potassium, chloride, and magnesium. It is used in the treatment of several disorders including edema, hypertension, diabetes insipidus, and hypoparathyroidism.

In 2023, this provider wrote 12 Medicare prescriptions for Hydrochlorothiazide, about 14% of the volume of the typical South Carolina prescriber of this medication (84). Medicare paid an average of $4.80 per fill, versus $4.86 statewide.

About this data. The prescription history for LATOYA DENISE MICKLE APRN is compiled from the Centers for Medicare & Medicaid Services (CMS) Medicare Part D Prescriber Public Use File for calendar year 2023, the most recent federal release. Figures cover prescriptions filled for Medicare Part D beneficiaries only; prescriptions under private insurance, Medicaid, or self-pay plans are not included. Because prescribing decisions are generally consistent across coverage types, this record offers a useful view of the overall Family prescribing approach at this Columbia, SC practice.

Reading the costs. Drug costs are gross amounts paid to pharmacies under Part D, including both plan and beneficiary payments. They are not the price a patient pays at the pharmacy counter. Medications with fewer than 11 claims are excluded from the federal source data, and state comparisons use the average volume among SC prescribers of the same medication. Always confirm current medications and costs directly with the provider and your plan.
