How to read a CLIA number
A CLIA number is a unique 10-character identifier that CMS assigns to every certified laboratory. It is not random: the characters follow a fixed pattern that tells you where the lab is and how its certificate is tracked.

Reading the example 45D0995241 from left to right: the first two digits (45) are the numeric state code CMS uses to identify where the laboratory is located. In this example, that state is Texas. The third character is almost always the letter D, which designates the record as a CLIA certificate. The final seven digits (0995241) are the unique sequence that identifies the individual facility. When you enter or transmit a CLIA number, include all ten characters exactly as shown.
How do I find a lab's CLIA number?

What does the state code at the start mean?
The first two digits are a numeric code CMS assigns to each state and territory. They identify where the laboratory is physically located, which is why a search by state narrows results to labs whose CLIA numbers share that prefix.
Is a CLIA number the same as an NPI?
No. They are separate identifiers maintained by different systems. The NPI identifies the provider in the national registry; the CLIA number identifies the laboratory's certification. A single lab usually has both, which is what this crosswalk links together.
