This medication is also used to treat other types of thyroid disorders (such as thyroid cancer). This medication should not be used to treat infertility unless it is caused by low thyroid hormone levels. The major hormone derived from the thyroid gland. Thyroxine is synthesized via the iodination of tyrosines (MONOIODOTYROSINE) and the coupling of iodotyrosines (DIIODOTYROSINE) in the THYROGLOBULIN. Thyroxine is released from thyroglobulin by proteolysis and secreted into the blood. Thyroxine is peripherally deiodinated to form TRIIODOTHYRONINE which exerts a broad spectrum of stimulatory effects on cell metabolism.

About Fluticasone-Salmeterol: This product is used to control and prevent symptoms (wheezing and shortness of breath) caused by asthma or ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

It contains 2 medications: fluticasone and salmeterol. Fluticasone belongs to a class of drugs known as corticosteroids. It works by reducing the irritation and swelling of the airways. Salmeterol belongs to the class of drugs known as long-acting beta agonists. It works by relaxing the muscles around the airways so that they open up and you can breathe more easily. Controlling symptoms of breathing problems can decrease time lost from work or school. When used alone, long-acting beta agonists (such as salmeterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination inhaled corticosteroid and long-acting beta agonists, such as this product, do not increase the risk of serious asthma-related breathing problems. For asthma treatment, this product should be used when breathing problems are not well controlled with one asthma-control medication (such as inhaled corticosteroid) or if your symptoms need combination treatment. Before using this medication, it is important to learn how to use it properly. This medication must be used regularly to be effective. It does not work right away and should not be used to relieve sudden asthma attacks. If an asthma attack occurs, use your quick-relief inhaler (such as albuterol, also called salbutamol in some countries) as prescribed. This medication also contains an ingredient that helps to make the urine acidic. When the urine is acidic, methenamine turns into formaldehyde to kill the bacteria. This drug is effective only against bacterial infections in the urinary tract. It will not work for other types of bacterial infections (such as in the blood) or for viral infections (such as the common cold or flu). Unnecessary use or misuse of any antibacterial drug can lead to its decreased effectiveness. Both parts of molecule contribute to its antibacterial action.

In 2024, this provider wrote 13 Medicare prescriptions for Methenamine Hippurate, about a third of the volume of the typical Nebraska prescriber of this medication (34.7). Medicare paid an average of $44.50 per fill, versus $60.48 statewide.

About Gemfibrozil: Gemfibrozil is used along with a proper diet to help lower fats (triglycerides) and raise "good" cholesterol (HDL) in the blood. Show more ▾

It may also help to lower "bad" cholesterol (LDL). Gemfibrozil belongs to a group of drugs known as "fibrates." It works by decreasing the amount of fat produced by the liver. Lowering triglycerides in people with very high triglyceride blood levels may decrease the risk of pancreas disease (pancreatitis). However, gemfibrozil might not lower your risk of a heart attack or stroke. Talk to your doctor about the risk and benefits of gemfibrozil. In addition to eating a proper diet (such as a low-cholesterol/low-fat diet), other lifestyle changes that may help this medication work better include exercising, drinking less alcohol, losing weight if overweight, and stopping smoking. A lipid-regulating agent that lowers elevated serum lipids primarily by decreasing serum triglycerides with a variable reduction in total cholesterol.

About Trelegy Ellipta: This product is used to control and prevent symptoms (such as wheezing and shortness of breath) caused by asthma and ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

Controlling symptoms of breathing problems helps you stay active. This inhaler contains 3 medications: fluticasone, umeclidinium, and vilanterol. Fluticasone belongs to a class of drugs known as corticosteroids. It works by reducing swelling of the airways in the lungs to make breathing easier. Umeclidinium belongs to a class of drugs known as anticholinergics and vilanterol is a LABA medication. Both drugs work by relaxing the muscles around the airways so that they open up and you can breathe more easily. Both drugs are also known as bronchodilators. When used alone, long-acting beta agonists (such as vilanterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination products containing both an inhaled corticosteroid and long-acting beta agonist, such as this product, do not increase the risk of serious asthma-related breathing problems. For asthma treatment, this product should be used when breathing problems are not well controlled with two asthma-control medications (such as inhaled corticosteroid and long-acting beta agonist) or if your symptoms need combination treatment. This medication must be used regularly to be effective. It does not work right away and should not be used to relieve sudden shortness of breath. If sudden breathing problems occur, use your quick-relief inhaler as prescribed.

In 2024, this provider wrote 12 Medicare prescriptions for Trelegy Ellipta, about a quarter of the volume of the typical Nebraska prescriber of this medication (48.5). Medicare paid an average of $679.84 per fill, versus $756.12 statewide.

About Trulicity: Dulaglutide is used with a proper diet and exercise program to control high blood sugar in people with type 2 diabetes. Show more ▾

Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. This medication is also used to lessen the risk of a major cardiovascular event (such as heart attack or stroke) in people who already have, or are at high risk for heart/blood vessel disease. Dulaglutide is similar to a natural hormone in your body (incretin). It works by causing insulin release in response to high blood sugar (such as after a meal) and by decreasing the amount of sugar your liver makes. Dulaglutide is not a substitute for insulin if you need insulin treatment. A glucagon-like peptide-1 receptor agonist; GLP-1 immunoglobulin G (IgG4) Fc fusion protein with extended activity.

About this data. The prescription history for EDWARD P MONTANEZ M.D. is compiled from the Centers for Medicare & Medicaid Services (CMS) Medicare Part D Prescriber Public Use File for calendar year 2024, the most recent federal release. Figures cover prescriptions filled for Medicare Part D beneficiaries only; prescriptions under private insurance, Medicaid, or self-pay plans are not included. Because prescribing decisions are generally consistent across coverage types, this record offers a useful view of the overall Family Medicine prescribing approach at this Elkhorn, NE practice.

Reading the costs. Drug costs are gross amounts paid to pharmacies under Part D, including both plan and beneficiary payments. They are not the price a patient pays at the pharmacy counter. Medications with fewer than 11 claims are excluded from the federal source data, and state comparisons use the average among NE prescribers of the same medication and form, shown only when at least five prescribers contribute. Always confirm current medications and costs directly with the provider and your plan.

About the medication summaries. Drug use descriptions come from the CMS Medicare Part D Drug Spending Dashboard (2024), and substance definitions from Medical Subject Headings (MeSH) 2026, U.S. National Library of Medicine. They are summaries that do not contain all possible information about a medication and are not individual medical advice. Ask a health care professional about any medication and your specific health needs.
