About Levothyroxine Sodium: Levothyroxine is used to treat an underactive thyroid (hypothyroidism). Show more ▾

It replaces or provides more thyroid hormone, which is normally produced by the thyroid gland. Low thyroid hormone levels can occur naturally or when the thyroid gland is injured by radiation/medications or removed by surgery. This medication is also used to treat other types of thyroid disorders (such as thyroid cancer). This medication should not be used to treat infertility unless it is caused by low thyroid hormone levels. The major hormone derived from the thyroid gland. Thyroxine is synthesized via the iodination of tyrosines (MONOIODOTYROSINE) and the coupling of iodotyrosines (DIIODOTYROSINE) in the THYROGLOBULIN. Thyroxine is released from thyroglobulin by proteolysis and secreted into the blood. Thyroxine is peripherally deiodinated to form TRIIODOTHYRONINE which exerts a broad spectrum of stimulatory effects on cell metabolism.

About Fluticasone-Salmeterol: This product is used to control and prevent symptoms (wheezing and shortness of breath) caused by asthma or ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

It contains 2 medications: fluticasone and salmeterol. Fluticasone belongs to a class of drugs known as corticosteroids. It works by reducing the irritation and swelling of the airways. Salmeterol belongs to the class of drugs known as long-acting beta agonists. It works by relaxing the muscles around the airways so that they open up and you can breathe more easily. Controlling symptoms of breathing problems can decrease time lost from work or school. When used alone, long-acting beta agonists (such as salmeterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination inhaled corticosteroid and long-acting beta agonists, such as this product, do not increase the risk of serious asthma-related breathing problems. For asthma treatment, this product should be used when breathing problems are not well controlled with one asthma-control medication (such as inhaled corticosteroid) or if your symptoms need combination treatment. Before using this medication, it is important to learn how to use it properly. This medication must be used regularly to be effective. It does not work right away and should not be used to relieve sudden asthma attacks. If an asthma attack occurs, use your quick-relief inhaler (such as albuterol, also called salbutamol in some countries) as prescribed.

In 2023, this provider wrote 38 Medicare prescriptions for Fluticasone-Salmeterol, about 1.3 times the volume of the typical Florida prescriber of this medication (30). Medicare paid an average of $231.50 per fill, versus $214.18 statewide.

About Vitamin D2: Vitamin D (ergocalciferol-D2, cholecalciferol-D3, alfacalcidol) is a fat-soluble vitamin that helps your body absorb calcium and phosphorus. Show more ▾

Having the right amount of vitamin D, calcium, and phosphorus is important for building and keeping strong bones. Vitamin D is used to treat and prevent bone disorders (such as rickets, osteomalacia). Vitamin D is made by the body when skin is exposed to sunlight. Sunscreen, protective clothing, limited exposure to sunlight, dark skin, and age may prevent getting enough vitamin D from the sun. Vitamin D with calcium is used to treat or prevent bone loss (osteoporosis). Vitamin D is also used with other medications to treat low levels of calcium or phosphate caused by certain disorders (such as hypoparathyroidism, pseudohypoparathyroidism, familial hypophosphatemia). It may be used in kidney disease to keep calcium levels normal and allow normal bone growth. Vitamin D drops (or other supplements) are given to breast-fed infants because breast milk usually has low levels of vitamin D. Derivatives of ERGOSTEROL formed by ULTRAVIOLET RAYS breaking of the C9-C10 bond. They differ from CHOLECALCIFEROL in having a double bond between C22 and C23 and a methyl group at C24.

About Ozempic: Semaglutide is used with a proper diet and exercise program to control high blood sugar in people with type 2 diabetes. Show more ▾

Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Semaglutide is also used in people with type 2 diabetes and heart disease to lower the risk of death from heart attack or stroke. Semaglutide is similar to a natural hormone in your body (incretin). It works by causing insulin release in response to high blood sugar (such as after a meal) and decreasing the amount of sugar your liver makes. If you use insulin, semaglutide is not a substitute for insulin treatment.

About Warfarin Sodium: This medication is used to treat blood clots (such as in deep vein thrombosis-DVT or pulmonary embolus-PE) and/or to prevent new clots from forming in your body. Show more ▾

Preventing harmful blood clots helps to reduce the risk of a stroke or heart attack. Conditions that increase your risk of developing blood clots include a certain type of irregular heart rhythm (atrial fibrillation), heart valve replacement, recent heart attack, and certain surgeries (such as hip/knee replacement). Warfarin is commonly called a blood thinner, but the more correct term is anticoagulant. It helps to keep blood flowing smoothly in your body by decreasing the amount of certain substances (clotting proteins) in your blood. An anticoagulant that acts by inhibiting the synthesis of vitamin K-dependent coagulation factors. Warfarin is indicated for the prophylaxis and/or treatment of venous thrombosis and its extension, pulmonary embolism, and atrial fibrillation with embolization. It is also used as an adjunct in the prophylaxis of systemic embolism after myocardial infarction. Warfarin is also used as a rodenticide.

In 2023, this provider wrote 23 Medicare prescriptions for Warfarin Sodium, about 62% of the volume of the typical Florida prescriber of this medication (37). Medicare paid an average of $7.29 per fill, versus $12.70 statewide.

About Trelegy Ellipta: This product is used to control and prevent symptoms (such as wheezing and shortness of breath) caused by asthma and ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

Controlling symptoms of breathing problems helps you stay active. This inhaler contains 3 medications: fluticasone, umeclidinium, and vilanterol. Fluticasone belongs to a class of drugs known as corticosteroids. It works by reducing swelling of the airways in the lungs to make breathing easier. Umeclidinium belongs to a class of drugs known as anticholinergics and vilanterol is a LABA medication. Both drugs work by relaxing the muscles around the airways so that they open up and you can breathe more easily. Both drugs are also known as bronchodilators. When used alone, long-acting beta agonists (such as vilanterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination products containing both an inhaled corticosteroid and long-acting beta agonist, such as this product, do not increase the risk of serious asthma-related breathing problems. For asthma treatment, this product should be used when breathing problems are not well controlled with two asthma-control medications (such as inhaled corticosteroid and long-acting beta agonist) or if your symptoms need combination treatment. This medication must be used regularly to be effective. It does not work right away and should not be used to relieve sudden shortness of breath. If sudden breathing problems occur, use your quick-relief inhaler as prescribed.

In 2023, this provider wrote 22 Medicare prescriptions for Trelegy Ellipta, about a third of the volume of the typical Florida prescriber of this medication (55). Medicare paid an average of $704.14 per fill, versus $900.09 statewide.

About this data. The prescription history for JAY PETER LAO NP is compiled from the Centers for Medicare & Medicaid Services (CMS) Medicare Part D Prescriber Public Use File for calendar year 2023, the most recent federal release. Figures cover prescriptions filled for Medicare Part D beneficiaries only; prescriptions under private insurance, Medicaid, or self-pay plans are not included. Because prescribing decisions are generally consistent across coverage types, this record offers a useful view of the overall Gerontology prescribing approach at this Bradenton, FL practice.

Reading the costs. Drug costs are gross amounts paid to pharmacies under Part D, including both plan and beneficiary payments. They are not the price a patient pays at the pharmacy counter. Medications with fewer than 11 claims are excluded from the federal source data, and state comparisons use the average volume among FL prescribers of the same medication. Always confirm current medications and costs directly with the provider and your plan.
