About Hydrochlorothiazide: This medication is used to treat high blood pressure. Show more ▾

Lowering high blood pressure helps prevent strokes, heart attacks, and kidney problems. Hydrochlorothiazide belongs to a class of drugs known as diuretics/"water pills." It works by causing you to make more urine. This helps your body get rid of extra salt and water. This can lessen symptoms such as shortness of breath or swelling in your ankles or feet. A thiazide diuretic often considered the prototypical member of this class. It reduces the reabsorption of electrolytes from the renal tubules. This results in increased excretion of water and electrolytes, including sodium, potassium, chloride, and magnesium. It is used in the treatment of several disorders including edema, hypertension, diabetes insipidus, and hypoparathyroidism.

In 2024, this provider wrote 30 Medicare prescriptions for Hydrochlorothiazide, about a third of the volume of the typical Missouri prescriber of this medication (85.1). Medicare paid an average of $2.42 per fill, versus $3.84 statewide.

About Levothyroxine Sodium: Levothyroxine is used to treat an underactive thyroid (hypothyroidism). Show more ▾

It replaces or provides more thyroid hormone, which is normally produced by the thyroid gland. Low thyroid hormone levels can occur naturally or when the thyroid gland is injured by radiation/medications or removed by surgery. Having enough thyroid hormone is important for maintaining normal mental and physical activity. In children, having enough thyroid hormone is important for normal mental and physical development. This medication is also used to treat other types of thyroid disorders (such as thyroid cancer). This medication should not be used to treat infertility unless it is caused by low thyroid hormone levels. The major hormone derived from the thyroid gland. Thyroxine is synthesized via the iodination of tyrosines (MONOIODOTYROSINE) and the coupling of iodotyrosines (DIIODOTYROSINE) in the THYROGLOBULIN. Thyroxine is released from thyroglobulin by proteolysis and secreted into the blood. Thyroxine is peripherally deiodinated to form TRIIODOTHYRONINE which exerts a broad spectrum of stimulatory effects on cell metabolism.

In 2024, this provider wrote 22 Medicare prescriptions for Levothyroxine Sodium, about 11% of the volume of the typical Missouri prescriber of this medication (193.8). Medicare paid an average of $8.43 per fill, versus $8.86 statewide.

About Promethazine Hcl: See also Warning section. Show more ▾

Promethazine is used to prevent and treat nausea and vomiting related to certain conditions (such as before/after surgery, motion sickness). It is also used to treat allergy symptoms such as rash, itching, and runny nose. It may be used to help you feel sleepy/relaxed before and after surgery or to help certain opioid pain relievers (such as meperidine) work better. It may also be used for a short time to treat a runny nose due to the common cold. Promethazine is an antihistamine and works by blocking a certain natural substance (histamine) that your body makes during an allergic reaction. Its other effects (such as anti-nausea, calming, pain relief) may work by affecting other natural substances (such as acetylcholine) and by acting directly on certain parts of the brain. Cough-and-cold products have not been shown to be safe or effective in children younger than 6 years. Do not use this product to treat cold symptoms in children younger than 6 years unless specifically directed by the doctor. Some products (such as long-acting tablets/capsules) are not recommended for use in children younger than 12 years. Ask your doctor or pharmacist for more details about using your product safely. These products do not cure or shorten the length of the common cold and may cause serious side effects. To decrease the risk for serious side effects, carefully follow all dosage directions. Do not give other cough-and-cold medication that might contain the same or similar ingredients (see also Drug Interactions section). Ask the doctor or pharmacist about other ways to relieve cough and cold symptoms (such as drinking enough fluids, using a humidifier or saline nose drops/spray). A phenothiazine derivative with histamine H1-blocking, antimuscarinic, and sedative properties. It is used as an antiallergic, in pruritus, for motion sickness and sedation, and also in animals.

About Symbicort: This product is used to control and prevent symptoms (wheezing and shortness of breath) caused by asthma or ongoing lung disease (chronic obstructive pulmonary disease-COPD, which includes chronic bronchitis and emphysema). Show more ▾

It contains 2 medications: budesonide and formoterol. Budesonide belongs to a class of drugs known as corticosteroids. It works by reducing the irritation and swelling of the airways. Formoterol belongs to the class of drugs known as long-acting beta agonists. It works by relaxing the muscles around the airways so that they open up and you can breathe more easily. Controlling symptoms of breathing problems can decrease time lost from work or school. When used alone, long-acting beta agonists (such as formoterol) may rarely increase the risk of serious (sometimes fatal) asthma-related breathing problems. However, combination inhaled corticosteroid and long-acting beta agonists, such as this product, do not increase the risk of serious asthma-related breathing problems. Before using this medication, it is important to learn how to use it properly. If an asthma attack occurs, use your quick-relief inhaler (such as albuterol, also called salbutamol in some countries) as prescribed. See also How to Use section. About Eliquis: Apixaban is used to prevent serious blood clots from forming due to a certain irregular heartbeat (atrial fibrillation) or after hip/knee replacement surgery. Show more ▾

About this data. The prescription history for SHURUK HAIB M.D is compiled from the Centers for Medicare & Medicaid Services (CMS) Medicare Part D Prescriber Public Use File for calendar year 2024, the most recent federal release. Figures cover prescriptions filled for Medicare Part D beneficiaries only; prescriptions under private insurance, Medicaid, or self-pay plans are not included. Because prescribing decisions are generally consistent across coverage types, this record offers a useful view of the overall Family Medicine prescribing approach at this Joplin, MO practice.

Reading the costs. Drug costs are gross amounts paid to pharmacies under Part D, including both plan and beneficiary payments. They are not the price a patient pays at the pharmacy counter. Medications with fewer than 11 claims are excluded from the federal source data, and state comparisons use the average among MO prescribers of the same medication and form, shown only when at least five prescribers contribute. Always confirm current medications and costs directly with the provider and your plan.

About the medication summaries. Drug use descriptions come from the CMS Medicare Part D Drug Spending Dashboard (2024), and substance definitions from Medical Subject Headings (MeSH) 2026, U.S. National Library of Medicine. They are summaries that do not contain all possible information about a medication and are not individual medical advice. Ask a health care professional about any medication and your specific health needs.
