Hope is phenomenal Case Manager. She currently offers services in Targeted Case Management and 1915i Care Coordination. She provides person-centered care and builds effective care teams for the people she serves. Hope has positively impacted the lives of hundreds of individuals in Fargo and the surrounding area. She offers Adult and Child Case Management.
